INSTRUCTIONS FOR SUBMITTING A CERTIFICATE OF INSURANCE REQUEST:

A completed request form can be sent to us in one of two ways:

1) Print the completed form and fax to 607-796-5591

OR

2) When exiting the completed form (by clicking on the x in the top right corner of the window), you will be asked if you would like to save your changes. Click "yes" to save the document and select a location on your computer to save it.  You can also rename the document if you would like for your records.  Next, send us an email with the saved file attached to it.  Email the request to sstadelmaier@wjfarmerinsurance.com. 

Please be sure to complete all sections of the form!
